A major study by McCormick et al <\/span><\/span>[<\/span>1]<\/span>\u00a0published in\u00a0<\/span><\/span>The Lancet Global Health<\/span><\/span><\/em>\u00a0<\/span>earlier this year<\/span><\/span>\u00a0<\/span>depicts a<\/span>\u00a0drasti<\/span>c<\/span>\u00a0treatment gap,<\/span>\u00a0mak<\/span>ing a clear<\/span>\u00a0case<\/span>\u00a0<\/span>for exactly the kind of solution\u00a0<\/span>programa<\/span>\u00a0has been building for more than a decade.<\/span><\/span>\u00a0<\/span><\/p>\n

<\/p>\n

A Global Target At Risk<\/strong><\/h3>\n

In 2021, member states at the 74th World Health Assembly\u00a0of the World Health\u00a0Organization\u00a0endorsed\u00a0the\u00a0ambitious goal\u00a0of\u00a0a 30%\u00a0global\u00a0increase in\u00a0<\/span>effective cataract surgical coverage (eCSC)<\/span>\u00a0by 2030. Unlike crude surgical volume,\u00a0eCSC\u00a0is a quality-adjusted metric\u00a0measuring\u00a0the proportion of people who received cataract surgery\u00a0<\/span>y<\/span>\u00a0<\/span><\/i>achieved a good visual outcome as a share of all those who have had or still need surgery. In essence,\u00a0eCSC\u00a0is\u00a0a measure of whether cataract care is working.<\/span>\u00a0<\/span><\/p>\n

The\u00a0latest study in\u00a0<\/span>Lancet Global Health [1]<\/span><\/i>\u00a0considered\u00a0a secondary analysis of 233 population-based surveys across 68 countries from 2003 to 2024. Researchers\u00a0found the world is\u00a0far from\u00a0on track to meet the\u00a0goal.\u00a0In fact, country-level\u00a0eCSC\u00a0estimates ranged from just 2.1% in Burundi to 77.7% in Qatar. Globally\u00a0in 2025,\u00a0eCSC\u00a0was estimated at 48.2%.\u00a0\u00a0<\/span>\u00a0<\/span><\/p>\n

Sobering Realities<\/strong><\/h3>\n

Results of the study underscore\u00a0that\u00a0more than half of people who need cataract surgery\u00a0do\u00a0not receive\u00a0care that restores\u00a0good\u00a0vision. The study’s modeled projections suggest global\u00a0eCSC\u00a0will increase by only 8.4%\u00a0by 2030,\u00a0falling dramatically short of the target.<\/span>\u00a0<\/span><\/p>\n

The regional picture is particularly troubling in Africa, where\u00a0eCSC\u00a0was projected\u00a0at\u00a023.6% in 2025 and is expected to rise by only 4% in the next 4 years.\u00a0The study also found women face significantly lower\u00a0eCSC\u00a0than men.<\/span>\u00a0<\/span><\/p>\n

These numbers do more than reveal a statistical shortfall. <\/span>\u00a0They represent hundreds of millions of people living with preventable blindness or visual impairment.<\/span>\u00a0<\/span><\/p>\n

Quality Maters as Much as Access<\/h3>\n

The\u00a0study\u00a0also\u00a0analyzed\u00a0data from 120 surveys\u00a0to\u00a0determine\u00a0several\u00a0drivers\u00a0of\u00a0poor outcomes in people who\u00a0undergo cataract\u00a0surgery.\u00a0One crucial driver\u00a0found\u00a0by researchers is that\u00a0intraoperative complications accounted for 20.2% of suboptimal outcomes across all regions.\u00a0<\/span>\u00a0<\/span><\/p>\n

The implication is dire: A substantial portion of the global quality gap in cataract outcomes may be attributed to complications that arise during surgery. Better surgical training may likely translate to fewer intraoperative errors. A multicenter, investigator-masked randomized controlled trial found novice surgeons who trained with a HelpMeSee Eye Surgery Simulator made significantly fewer errors during their first 20 live manual small incision cataract surgery procedures compared with surgeons who received conventional training alone. [2]<\/span>\u00a0<\/span><\/p>\n

A parallel, randomized, controlled, multicenter trial across five university hospitals in four countries found that participants who underwent virtual reality simulation training scored 16.6 points higher at 3 months than participants in the control group and performed a mean of 21.5 cataract surgical procedures in the year after training, compared to 8.5 in the control group. Posterior capsule rupture rates were also lower for the training group (7.8% vs 26.6%). [3]<\/span>\u00a0<\/span>\u00a0<\/span><\/p>\n

Bridging the Training Gap<\/strong><\/h3>\n

T<\/span>he shortage of skilled<\/span>\u00a0cataratas<\/span>\u00a0surgeons<\/span>,\u00a0<\/span>particularly in low- and middle-income countries<\/span>,\u00a0<\/span>is\u00a0<\/span>a<\/span>n<\/span>\u00a0<\/span>obstacle<\/span>\u00a0a<\/span>ward<\/span>\u00a0progress.\u00a0<\/span>programa<\/span>\u00a0has a global mission to eradicate cataract blindness by increasing\u00a0<\/span>training in MSICS, a safe, reliable procedure that delivers successful outcomes at\u00a0<\/span>low cost. MSICS is the surgical technique best suited\u00a0<\/span>for<\/span>\u00a0high-volume, resource-efficient cataract care in exactly the settings where the\u00a0<\/span>McCormick et al\u00a0<\/span>study\u00a0<\/span>identifies<\/span>\u00a0the greatest unmet need.<\/span><\/span>\u00a0<\/span><\/p>\n

The validity and reliability of the\u00a0HelpMeSee\u00a0Eye Surgery Simulator have also been\u00a0established\u00a0in independent research.\u00a0A study showed\u00a0most (94.3%)\u00a0experienced MSICS surgeons\u00a0felt\u00a0the simulator would be useful in developing hand-eye coordination and would recommend cataract surgical training on the\u00a0device. [4]<\/span> A separate study confirmed the simulator\u2019s ability to reliably differentiate between novice and experienced surgeons. [5]<\/span><\/p>\n

Most recently, a prospective study confirmed the\u00a0HelpMeSee\u00a0MSICS module effectively differentiates surgical experience levels ranging from novice residents to experts,\u00a0further supporting its use for objective, skills-based surgical education. [6]<\/span> Ophthalmic trainees\u00a0who practiced cataract surgery techniques on\u00a0the\u00a0simulator demonstrated significant improvements in both surgical skills and confidence levels compared to peers without simulator training.\u00a0<\/span>\u00a0<\/span><\/p>\n

\u00a0A Scalable Training Model<\/span><\/h3>\n

El programa\u00a0<\/span>Lancet Global Health<\/span><\/i> study<\/span>\u00a0explicitly\u00a0asserts\u00a0that\u00a0better surgical outcomes and\u00a0a\u00a0dramatically\u00a0increased volume of surgery,\u00a0particularly in Africa, Southeast Asia, and the Eastern Mediterranean,\u00a0are\u00a0needed to\u00a0achieve a\u00a030%\u00a0global\u00a0increase in\u00a0<\/span>eCSC\u00a0by 2030<\/span>.\u00a0HelpMeSee\u00a0subsidizes\u00a0MSICS\u00a0training\u00a0in\u00a0an expanding geography including countries like\u00a0India, China, Madagascar, and Ghana\u00a0as well as the latest addition of Nigeria,\u00a0in February 2026.<\/span>7<\/span>\u00a0With more than 40\u00a0surgical\u00a0simulator\u00a0units\u00a0and 15 training centers worldwide,\u00a0the nonprofit organization\u00a0partners with governments, universities, and innovators to fight the global cataract blindness crisis\u00a0and contribute to closing the gap in cataract surgery\u00a0volume outlined in the current study.<\/span>\u00a0<\/span><\/p>\n

Further, HelpMeSee is currently rolling out a competency-based MSICS curriculum focusing on the comprehensive care of patients including, pre-operative, peri-operative and post-operative care. This system represents an integrated, quality-focused approach recommended by the <\/span>Lancet<\/span><\/i> study. [1]<\/span>\u00a0<\/span><\/p>\n

The Path Forward<\/span><\/h3>\n

El programa\u00a0<\/span>Lancet Global Health<\/span><\/i>\u00a0study concludes\u00a0that\u00a0without “immediate and substantial investment in cataract services,” the 2030 target will not be\u00a0met. [1]<\/span>\u00a0<\/span>\u00a0<\/span><\/p>\n

Effective cataract surgical coverage<\/span>\u00a0cannot improve without\u00a0better trained\u00a0cataract\u00a0surgeons\u00a0at\u00a0scale,\u00a0and\u00a0cannot be\u00a0trained\u00a0without simulation-based training systems that are\u00a0validated, effective, and accessible.\u00a0This\u00a0is the work\u00a0HelpMeSee\u00a0does every day.<\/span>\u00a0<\/span><\/p>\n
